Donor Pricing and Shipping Costs

All
donor semen is provided in 1.2 cc cryovials. Volume of the
specimen will vary between 0.5 and 1.2 milliliters depending
upon the format and sperm concentration. All vials are frozen
to provide upon thaw a minimum motile sperm concentration
of 20 million per cc and may be as high as 50 million per
cc or more. We will replace or refund any vial that is found
defective from these standards**. All donor semen is to be
paid for in advance of shipping unless billed through your
physicians office. This greatly reduces our overhead, which
helps us to keep our prices as low as possible. Click

All
donor specimens are frozen with the use of a TYB (test yolk
buffer) cryoprotectant to aid in the recovery from being frozen
and to provide the sperm with the appropriate environment.
Our TYB freezing media is antibiotic free, and uses glycerol
as the cryoprotectant.

| Donor Semen: |
|
ICI
specimens may be washed in HTF(human tubal fluid) media
only, or completely un-washed. This format is appropriate for home-vaginal,
intracervical(ICI) use, or for IVF use. Volume 0.7-1.2cc
|

| IUI
specimens have been washed through Pure Sperm and re-suspended
in HTF media prior to freezing. These units are IUI ready when thawed.
Prepared for intrauterine insemination, but also great for home-vaginal,
intracervical or IVF use. These units are concentrated into a lower
volume but with a higher motility. Volume 0.4-0.7cc. |

| All donors are
frozen in both of the above formats, but not all donors will be available
in both formats at all times. Please check for availability. ICI specimens
may be washed prior to insemination, but initial sperm motility must
be checked prior to washing. We are not responsible for sperm mortality
due to post thaw handling or washing. |

Return
Policy
If, for any reason you need to return un-used
frozen donor units to the lab, you must notify the lab of your intent,
and include a written note inside the shipping box, so that the
returning dewar is immediately checked for the returning units.
If you do not notify the lab of the returning units, you will assume
all risk of the units not being discovered and their subsequent
loss. If donor units are returned to the lab and the units have
already been destroyed by thawing out before they get back to us
and the dewar has been out in excess of 15 days, there is no refund
available. No refund or exchange credit will be issued for lost
units when returned without notification prior to their arrival
at the lab. Call 1-800-786-5251. Upon arrival, the donor units are
returned to liquid nitrogen storage. If the units are to be re-shipped
to you for use in your next cycle, you will only be billed a $25
handling fee for restocking the units into liquid storage inventory.
If the returned units are not reshipped within one month, you will
start to incur storage charges at the rate of $20 per month. Failure
to pay any storage charges incurred as they become due may result
in the destruction of specimens.
Refund
Policy
If you have purchased specimens prior to the
units coming out of quarantine, and the specimens have not left
our facility, we will make a complete refund within one week of
the original purchase, minus a $10 processing fee. If it has been
in excess of one week since original purchase of quarantined specimens
there will be a $25 inventory handling/restocking charge. If you
keep pre-purchased donor specimens (out of quarantine) stored here,
you can get a 75% refund if the units are turned back over to us
(returned) while the donor is still a listed donor on our current
donor listing. If you return/release 6 or more units of pre-purchased
donor specimens (out of quarantine) that have never left our facility
after the donor has been removed from our active donor listing,
you will receive a 50% refund. If a donor is no longer listed as
a donor for sale, and you return/release fewer than 6 units of pre-purchased
donor specimens (out of quarantine), there is no refund available.
Once units leave our facility we are unable to refund or exchange
for any reason other than if units are proven to not meet our guarantee
guidelines**. We cannot re-sell any donor specimens to another client
once the specimens have left our physical control for any amount
of time under any circumstances.
**Refund or replacement can only be made when proof
of specimen failure is provided in written format and the specimen
is retained and available for return to our facility at our option,
and a written specimen analysis report is provided to us detailing
the analysis results performed by qualified personnel. All claims
for refund due to specimen quality must be processed within 30 days
of specimen receipt as we have no control over outside storage facilities,
and should be reported to us as soon as is practical. Sperm count
guarantees apply only to specimens upon appropriate initial thaw
and prior to any additional handling or washing procedures that
may be performed at the descretion of the processing laboratory.
Qualified personnel include MDs, Med Techs, Embryologists, or Clinical
Laboratory Scientists licensed and qualified for high complexity
testing procedures through CLIA. We will not make refund on specimens
that are used for insemination that then claim the specimen was
faulty. We make every effort to assure that our specimens are processed
and shipped to provide you with good sperm and in good condition.

| We ship
in dry nitrogen dewars that are designed, and charged with nitrogen
coolant, to hold specimens frozen and ready to use for up to 14-18
days if kept upright and closed. You are renting this equipment when
an order is shipped to you. We **guarantee (see below) the dewar to
hold it's coolant charge for at least seven days after delivery to
you. These dewars are shipped in a cardboard box that is 12" by 12"
by 24", and weigh approximately 25 lbs. This makes shipping costly,
but allows you the maximum convenience in timing your inseminations.
You are allowed seven days rental of the dewar included in the price
of shipping in order to time your insemination(s) . If you keep the
dewar in excess of the seven days, you will be charged a daily dewar
rental fee of $10/day. The return of the dewar is included in the
shipping fee for all shipments within the United States only. If the
dewar is shipped to you via UPS, a call tag will be issued for its
pickup and return. If your shipment is made through DHL, an airbill
for its' return is included in the shipment. All you have to do is
call for a pick-up. We can ship from one to thirty vials at a time
for the same cost. For

| We prefer
to have a three or more business day advanced notice of need to deliver.
Deliveries can be made with shorter notice, but the cost can escalate
for type of service needed. Please give us the best estimate of time
needed as soon as possible in your cycle so as to simplify logistics
and reduce your costs. Please plan to have your specimens arrive 2-3
days prior to your anticipated ovulation date. This provides a cushion
of time in case you ovulate a day or two earlier than expected, and
yet the 7 days use of the dewar will still cover adequately if you
end up ovulating one or two days later than expected. If you order
on the same day that your specimens need to be shipped from our facility,
you will be charged a late order fee of $25 if placed prior to 12
pm, or an emergency rush order fee of $50 if placed after 12 pm but
before 3 pm. Please try to avoid placing your order for specimens
late, as it costs you more, and it makes it difficult for us. The
dewars that we ship with are expensive, and you must provide complete
client registration information or you will be required to place a
$500 refundable deposit with us prior to shipments. Dewar deposit
by credit card is not accepted. Dewar deposits can be made in advance
with US money order, cashiers checks through US banks, Western Union
funding, or by wire transfer of funding. Please call our office for

Shipping rates
Our preferred standard shipping requires two
business days for delivery within the lower 48 states and runs $139.
For same day shipping, all rush orders received between 8:30 - 12
PM will incur a $25 rush order surcharge so please plan ahead and
order before the day you need your order shipped. All orders
placed between 12PM and 3 PM for same day shipping will be termed
emergency shipments and are subject to a $50 surcharge. All pricing
below includes up to seven days rental of the dewar (once delivered)
in order to time the insemination. Additional days of dewar rental
are charged at $10/day. Deliveries scheduled for two day service
may sometimes be delivered next day even if you only paid for two
day service as the carrier will deliver as soon as the package reaches
the local delivery point. If you will not be available for receipt
of a shipment, you can by special request, order that the package
not be delivered early, or you can arrange to have the package held
by the carrier for you to pick up. We can not provide you with a
time of day for delivery. You may incur additional charges
or require additional delivery time if your delivery address is
incomplete, incorrect, remote, or in an outlying area.

#3 New
York State: Physicians, PAs, and Nurses performing
donor insemination in New York State are only able to use donor specimens
from banks that license themselves with the State of New York. New
York state has their own special licensure, and has created a complex
regulatory burden. As such, many US donor banks have chosen not to
obtain NY licensure. Unless using an out of state physician, this
effectively reduces the number of potential donors that a NY client
can choose from as it eliminates about half of the sperm bank donor
selections that clients in the rest of the USA can pick from. We are
not licensed through the State of New York, and do not plan to do
so at this time. Under some instances, if you are unable to find a
suitable donor through banks that are licensed with the state, your
NY doctor can submit paperwork for exemption which permits you to
use donor specimens from a bank that is not licensed through the state.
If such exemption is obtained, we have no problem shipping under such
exemption. |

International Shipping: Call or email for specific quotes on
all international rates. Please provide full address and postal
code.
All international shipping requires that the client
place a deposit on the shipping dewar that is refundable upon the
return of the undamaged dewar. Dewar
deposit by credit card is not accepted. Dewar deposits can be made
in advance with US money order, cashiers checks through US banks,
Western Union funding, or by wire transfer of funding. Please call
our office for instructions for Western Union pickup location or
for wire transfer information. The importer
(client) is responsible for any taxes and or duties that may be
associated with the import. Please plan ahead for international
orders as customs may hold packages for additional time, although
problems have been extremely rare. As the importer, you are responsible
for making sure that you meet all regulations for your country concerning
the imported product. We have recently shipped directly to clients
in Canada, Italy, Germany, the UK, Netherlands, Sweden, Mexico,
Brazil, Japan, and Australia, New Zealand, Scotland, Ireland, Singapore,
Switzerland, Panama, China, American Samoa,with no problems encountered.
We normally use DHL, the worlds largest international
carrier, to provide delivery service. Some examples of approximate
rates and delivery times are found below. All pricing is in US Funds.
All rates include up to 7 days rental of the shipping dewar once
delivered in order to hold the specimens frozen until use. International
shipping prices are for door to door one way service. The client
is responsible for return shipping and associated charges. We are
happy to quote exact shipping costs if you provide us with a full
delivery address via email.
Canada: $175 for most locations, approx. 2-3
day delivery time
Mexico: $210 for most
locations, approx. 2-3 day delivery time
South America: $275
for most locations, approx. 2-4 day delivery time
Europe: $195 and up, approx. 2-5 day delivery
time. Please submit exact and complete shipping address via email
for quoted price. (France and Spain have some restrictions on who
can import this commodity) We have not experienced any difficulty
in shipping into the UK.
Japan: $195 and up, approx. 2-5 day delivery
time. Please submit exact and complete shipping address via email
for quoted price. We have not experienced any difficulty in shipping
into Japan.
Australia: $225 and up, approx. 2-5 day delivery
time. Each state in Australia is different in their rules regarding
the import of donor semen. In NSW, Queensland, Australian Capital
Territory, and Tasmania there is no state legislation that we are
currently aware of, so there may not be any regulation of the import
of screened sperm, but you should check with clinics in those regions.
Victoria, West Australia, and South Australia do have legislation
concerning the import, so you would need to check with them to see
what the requirements are for each region. We were provided the
following contacts to obtain information regarding local regulation
concerning import of donor sperm specimens.

For example, in Victoria, the Infertility/Health Authorities
enforce the following rules for donor sperm specimen imports:
1-the donor must have been counseled by a Victorian approved infertility
counselor
2-there must be no commercial payment for the sperm (this eliminates
the use of our specimens, as well as specimens from any other commercial
bank)
3-identifyng information about the donor must
be lodged with the clinic where the treatment is to be undertaken,
so that in the event of a live birth, the donor's name and contact
details can be put on the Central Register
.
So, for example, we are unable to provide specimens into Victoria
because our donors are paid for their donations, and the client
pays for the specimens. Please make sure that you will be able to
get the shipment through customs. We have been able to successfully
ship direct to clinics in certain areas. Your clinic or local health
authorities should be able to advise you if import is possible.

** Dewar
guarantee: In the event of the loss or destruction of commercial
donor sperm specimens provided through NW Andrology due to the failure
of our shipping dewar to provide at least seven days worth of coolant,
(unless caused by the mishandling of the dewar by the client), Cryobank
will accomodate the clients loss by making
every reasonable effort to provide
the client with like or similar donor specimens if available, or
by refund of monies paid by client for vials lost (must be returned
to the Cryobank at original volume) plus shipping charges only.
